TJFL IN NO WAY GUARANTEES THE PLACEMENT OF ANY CHILD TO A SPECIFIC TEAM AND OR FIELD, TJFL ALSO WILL NOT MAKE ANY PLACEMENTS DUE TO TRANSPORTATION ISSUES. BY SIGNING BELOW I HAVE READ AND UNDERSTAND THIS STATEMENT.
By submiting this page for himself, his hears, representatives, successors, and for above named child, a minor child of whim each of the undersigned is either a parent or legal guardian, does hereby forever and absolutely waive and release any claim for damage or injury to the above - named minor child, which may be hereafter arise out of or result from participation by such child, at any time hereafter, in athletic or related activities of the THORNTON JUNIOR FOOTBALL LEAGUE ASSOCIATION (T.J.F.L.), an incorporated association whose activities are sometimes conducted on premises owned by the city of Thornton, CO., or any of its agents or employees, or any other person whose premises such activities may be conducted, or any other person acting under
the direction or control of such named parties, for any such claim. He consideration for the Release, Waiver, Covenant not to sue and Indemnity Agreement is the acceptance of minor child for participation in any of the activities conducted by any such named parties occurring on premises owned by any of such named parties, and it is acknowledged and agreed that such participation by such child, in any such activities , would be strictly forbidden absent the execution of this document by the undersigned. Each of the undersigned acknowledges that due to the nature of such activities there is a risk of injury, harm or even death, to the named minor child, and assumes all risk of any injury, harm, or death resulting from participation in such activities by such minor child.

You must agree to continue. I have full authority to execute this document, and have fully read it understand it and agree to the tearms and conditions.. it has been submited by legal parent, or guardian of the named child, and in consideration of acceptance of such child in such activities he or she will fully indemnify and hold harmless the named parties from any claim, loss, or damage (including reasonable attorneys fees incurred by the named parties in connection with any claim made by or on the behalf or, or arising out of, any injury to such named minor child), of any type or nature, including claims by or on the behalf or the undersigned, the named minor child, and any person with a right to present any claim of the undersigned, or of any such child, or of any parent, guardian, or custodian of the named minor child, absent willful wrong doing by any of the named parties. Parent or Guardian must provide T.J.F.L with a copy of Birth Certificate, and sports physical from licensed physician, for the above named minor child. I hereby grant permission to the T.J.F.L. representatives to authorize and obtain medical care form licensed medical professionals should the above named player become ill or injured while participating in league activities. I further grant permission to the T.J.F.L. to publish pictures of game action, which may or may not include photos of the above named child, for use on the T.J.F.L web page at www.tjfl.org or without any further compensation or permission.

I further grant permission to the T.J.F.L. to publish pictures of game action, which may or may not include photos of the above named child, for use on the T.J.F.L. web page at www.tjfl.org or without any further compensation or permission. I further agree to return all equipment supplied to the above named player by T.J.F.L., in as good of condition as when received, except for normal wear. In the event that equipment is lost, stolen, or not returned I agree to reimburse T.J.F.L. the sum of $300.00 for equipment plus any collection fees, attorney fees, and court Costs.
There will be a $25.00 bounce check fee, and the above named player will be held from participating until original check and bounce check fee is collected.
